Service Description
ADHD Clinical Assessment Service A supportive, structured pathway to clarity. Our ADHD assessments are designed to be straightforward, evidence-based and focused on practical next steps. What we’ll cover Background and history: academic concerns, health and medical history. Questionnaires: self-report measures exploring ADHD symptoms, mental wellbeing and health (with input from parents/guardians, lecturers or disability advisers where helpful). Cognitive testing: targeted assessments of working memory, cognitive functioning and attainment to build a rounded picture of strengths and challenges. Your four appointments (online or in person) Getting to Know You – a guided conversation about day-to-day/academic concerns and a review of the information gathered. Cognitive Testing – standardised assessments of thinking skills and learning profile. Diagnostic Interview – an in-depth discussion of ADHD symptoms across settings; a family member, partner or adviser can join to add perspective. Follow-Up – we go through the results together, answer questions, and agree next steps. What you receive A clear, plain-English report summarising findings and confirming the diagnostic outcome. Tailored recommendations for study, exams and daily life. Where appropriate, a referral letter to your GP, psychiatrist or ADHD specialist to discuss treatment options (including medication). Cancellation Policy
Cancellation Policy – Diagnostic, Clinical & Screening Assessments Applies to: Diagnostic Assessment, Clinical Assessment and Screening appointments at The Phoenix Psychology Practice. Cancellation window: Cancellations must be made at least 48 hours before the appointment. • Late cancellation fee: Where an appointment is not cancelled 48 hours before the start time, a £50 cancellation fee will be billed. • Rescheduling: Rescheduling must be agreed between the client and assessor with 72 hours’ notice. A rescheduling fee of £50.00 will be charged unless the change is due to an emergency (e.g., hospitalisation, acute illness, family crisis). • Provider cancellations: If the assessor is unavailable (e.g., illness), the Practice will give 24 hours’ notice wherever possible. A new appointment will be offered at no charge to the client. • Pre-appointment requirements: Clients are expected to complete all forms prior to their appointment. • Eye test: Where practicable, clients should have had a recent eye test before their assessment appointment. How to cancel or reschedule: Please use the Booking Enquiry Form or email emma@phoenix-psych.com with your name, appointment date/time, and your request. ________________________________________ Coaching & Counselling – Cancellation Policy Applies to: one-to-one coaching and counselling sessions (online or in person). Cancellation window: Cancellations must be made at least 48 hours before the appointment. • Late cancellation: Sessions cancelled with less than 48 hours’ notice are charged the full session fee. • Rescheduling: Changes requested with 72 hours’ notice can be accommodated subject to availability. A rescheduling fee [to verify amount] may apply unless due to an emergency (e.g., hospitalisation, acute illness, family crisis). • Provider cancellations: If we need to cancel, we will provide 24 hours’ notice wherever possible and offer a new appointment at no charge. • Late arrival: Sessions end at the scheduled time to respect following clients; fees apply to the booked time.
